Dear Resident:

Welcome to Central School District # 104, as we prepare for a great school year here are some things you can do to support our efforts. Make sure your child’s registration is current. Remember, ALL RETURNING AND NEW STUDENTS MUST REGISTER EACH SCHOOL YEAR. Obtain the necessary supplies for your child’s grade. The supply list can be found Click Here. Provide the necessary medical forms noted at the end of this letter.
If you didn’t register in May, registration will be held July 23rd and July 28th from 12:00pm-8:00pm and late registration will be held on August 6th from 9:00am-3:00pm in the Central School office. Students enrolling in both Joseph Arthur Middle School and Central must register at the Central School office at 309 Hartman Lane in O’Fallon.
The first day of student attendance for the 2009-2010 school year is August 14th. Your child must be registered before attending the first day of school. Because all school personnel will be very busy on the first day of school, NO STUDENTS will be registered on the first day. Additionally, students WILL NOT be assigned homerooms until they are registered.


STEP 1: PAPERWORK
Student Information Sheet – Please complete fully. Social Security Numbers will be required for all students.
Health History
Central School District 104 Authorization Form – Please read carefully and sign in all 5 places marked with an X.


STEP 2: RESIDENCY – (for new students or students that may have moved within the district)
One item from each of the following categories will need to be provided. If you cannot provide documentation from Category C, you may bring 2 items from Category B:
Category A
Home Ownership Title, Deed or Mortgage Statement
Lease Agreement
Current Real Estate Tax Bill
Category B
Current Utility Bill
Occupancy Permit
Homeowner or Renter Insurance Bill
Category C
Current paycheck stub with address in the District
Documentation of TANF or Approval Letter from Nutrition Program and Support Services
Driver’s License with address in the District
Students MUST reside within the boundaries of Central School District #104. Guardianship for the sole purpose of education will not be accepted. Any person who knowingly or willfully presents to any school district false information regarding residency of a pupil for the sole purpose of enabling that pupil to attend any school in that district without the payment of a non-resident tuition charge shall be guilty of a Class C misdemeanor, punishable by up to 30 days incarceration and/or a fine up to $1500.

STEP 3: REGISTRATION FEES
The school board has suspended registration fees for the 2009-2010 school year. A $10 fee will apply to any returning student who is registered after the August 6th deadline.


STEP 4: BUS PASS FEE
The fee for the bus pass is $5.00 for the school year. These must be available for the bus driver to check until they get to know the students.


STEP 5: FOOD SERVICE PROGRAM
There will be a representative from the Cafeteria to answer any questions and to take deposits on your child’s breakfast/lunch account.


ADDITIONAL ITEMS REQUIRED
Students registering that have not attended Central School in the past will be required to provide a Birth Certificate and Social Security Card.
Kindergarten, sixth grade, and new students from out-of-state must obtain a physical and proof of immunizations. These requirements must be submitted on or before the first day of the school term or within thirty days of enrollment.
Forms for recording student physical information are available at registration.
Any allergy, physical handicap, or health problem should be brought to the school’s attention at the time of registration.


Sixth, seventh, and eighth grade students participating in any of the school’s sports programs must have a sports physical prior to their trying out or practicing for the sport.
Dental Exams: HB 752 – Public Act 93-946 - Dental Exams are required for students in grades kindergarten, second, and sixth grade.
Eye Exams: SB0641 – Public Act 095-0671. An eye examination is required for students in Kindergarten and must be conducted by a licensed optometrist or a physician licensed to practice medicine in all of its branches.
