Please Note: This is a perfect opportunity for kindergarten, second and sixth graders to get the Illinois required dental examination completed by the May 15th, 2023 due date. ALL grade levels are invited and encouraged to participate as well, regardless of the Illinois grade level requirements. 2022-2023 HEALTH REQUIRMENTS

Incoming Early Childhood/Preschool students

The Illinois School Code(105 ILCS 5/27-8.1)requires that Incoming Early Childhood/Preschool students MUST have a current Illinois physical exam and an up-to-date immunization record on file for attendance to school. Now is a great time to schedule physical exam and immunization appointments so that your child will be ready to attend school in the fall of 2022. View the requirements in more detail in the left column under Documents .

Incoming Kindergarten students for the 2022-2023 school year

The Illinois School Code(105 ILCS 5/27-8.1)requires that kindergarten students MUST have a current Illinois physical exam and an up-to-date immunization record on file for attendance to school. Now is a great time to schedule physical and immunization appointments so that your child will be ready to attend kindergarten in the fall of 2022. The State of Illinois requires all incoming kindergarten students to have a current Illinois physical exam and up to date immunizations, vision exam and dental exam for entrance into kindergarten. View the requirements in more detail in the left column under Documents .

Students entering 6th grade for the 2022-2023 school year

Now is a great time to schedule physical and immunization appointments so that your child will be ready to attend sixth grade in the fall of 2022.
